If you are a NEW PATIENT, after you verify your account and create a username/password, the proper intake and consent forms will be sent to you prior to your appointment. These will be reviewed as part of your initial intake/exam/treatment. Please complete these forms. NOTE FOR NEW PATIENTS If INTAKE FORMS are NOT Completed PRIOR to your appointment, your appointment will be cancelled.
For “ all ” patients, please arrive no more than 5-10 minutes prior to your scheduled appointment. If you are running late, your appointment may be cancelled in order to maintain proper cleaning protocols between patients.
MASKS WILL BE REQUIRED AT ALL APPOINTMENTS
Please note that this office does not offer debit or credit services. All treatments must be paid for at the time of service via e-transfer, cash and/or insurance billing. The office is a paperless office for the environment and also to minimize contact. As a result, receipts will be e-mailed to you unless you ask specifically for print-outs.

Chiropractic is a non-invasive, hands-on health care discipline that focuses on the musculoskeletal system. Chiropractors practice a manual, hands on approach to muscular dysfunction, pain and injury by providing a diagnosis after a comprehensive evaluation. Treatment and preventive care for disorders related to the spine, pelvis, nervous system and joints is then recommended and may include referrals to other healthcare disciplines including but not limited to primary care physicians (family doctors), physiotherapists, dieticians etc. or modification of activity and even referral for additional imaging studies to be completed (Xrays, CT scans, MRI, diagnostic ultrasounds etc.). Dr. Reena Pathak will work with your other health care providers to enhance your treatment when necessary.
Chiropractors use a combination of treatments, all of which are predicated on the specific needs of the individual patient. For many conditions, such as lower back pain, chiropractic care is frequently the primary method of treatment. Where other conditions exist, chiropractic care may complement or support medical treatment by relieving the musculoskeletal aspects associated with the condition.
